CD Audio connection pin outs

Board revisions up to Pinnacle Rev E and Fiji Rev B have the correct 
SONY header, but the Panasonic connector's left and right channels 
are reversed:

    SONY:      L-G-G-R
    Panasonic: G-R-G-L

Board revisions Pinnacle Rev F and Fiji Rev C have the correct SONY and 
Panasonic connectors:

    SONY:      L-G-G-R
    Panasonic: G-L-G-R

Additionally, the SONY header has been changed from symmetrical to notched,
so the cable can't be accidentally plugged in the wrong way.
