First thing you want to try is using the 'stock' cable or the 'communicator' that came with your Xbox just to verify that the Xbox Controller is functioning properly. If the original Xbox cable is working, you know you have a cable that has failed. Since these cables have to be lightweight so that they do not "weigh you down" while playing, caution should be exercised not to get them caught under furniture etc.
You may also run the PC test available here (NOTE: This test will verify the issue with chat and double as a way to verify the setup on the PC, ensuring the mic is selected as default, etc.)
